Below are office forms used at our office, other than our intake packets. For the release form, please fill out the name of the person or practice with whom we will be releasing/sharing information, as well as the information to be released. Please sign and date the release form either as the client, or an authorized representative.

Doctors/ Referral Providers: For the referral form, please fill out our form(or if your practice has one that they use, feel free to use it instead) and fax it to our office. Please include the patient's insurance card, and last office visit note. As much detail as to why the patient is being referred to our office, will be much appreciated.
